背景:颅内多发性动脉瘤患者破裂部位的识别主要基于动脉瘤的大小、位置和形状。最新研究显示,磁共振(MR)血管壁成像呈现动脉瘤壁环形增强(CEAW)提示为破裂动脉瘤。
BACKGROUND:Identificationofrupturesitesinpatientswithmultipleintracranialaneurysmsislargelybasedonaneurysmsize,location,andshape.Findingcircumferentialenhancementalongtheaneurysmwall(CEAW)onmagneticresonance(MR)vesselwallimagingwasrecentlyshowntobeindicativeofrupturedaneurysm.
目的:探讨较高程度的CEAW能否识别多发性动脉瘤患者的破裂部位。
OBJECTIVE:ToinvestigatethehypothesisthatahigherdegreeofCEAWwouldidentifythesiteofruptureinpatientswithmultipleaneurysms.
方法:作者连续选取发生动脉瘤性蛛网膜下腔出血的多发性动脉瘤患者(26例患者,共计62枚动脉瘤),应用MR血管壁成像前瞻性地进行CEAW定量分析。在注射对比剂前后分别获取三维T1加权快速自旋回波序列,并且计算动脉瘤壁强化指数(WEI)。比较破裂与未破裂动脉瘤的特征(大小、位置、不规则形状、纵横比[瘤颈至瘤顶的长度/瘤颈宽度]和WEI),通过单因素条件性logistic回归分析计算动脉瘤破裂的比值比及95%置信区间,并且经动脉瘤大小校正后再次进行分析。
