年8月11-14日，中国心脏大会（CHC）在国家会议中心隆重召开，在11日的AATS瓣膜峰会上，中国医学科学院国家医院血管外科中心于存涛教授对主动脉夹层的阜外分型做了详细介绍。

主动脉夹层是危及病人生命最严重、最复杂的心血管疾病之一。随着全世界主动脉夹层发生率的讨论可知道，在冰岛每年发生率为2.53/000。台湾每年发生率为5.6/000。河北和北京在年的总人口数为95.95百万，预估最低发生率1.2/000。对于主动脉夹层的治疗首先要考虑主动脉夹层的解剖、临床表现及病理基础，这也是对主动脉进行分型的根本原则。

年的Debakey分型主要用于针对夹层累及的范围进行分型。年的Stanford分型有助于临床决策，其中A型需要紧急手术，B型夹层可以用药物治疗。为精确手术选择，于存涛教授研究组提出了主动脉阜外分型，以此明确病情缓急轻重，提供临床治疗策略，预测夹层治疗效果，最终能够达到精确部分分型盲区，细化外科治疗方案，改善夹层治疗预后的效果。

主动脉夹层阜外分型

1.分型依据：

夹层累及的主动脉解剖部位及范围。

2.分型方法：

根据夹层累及的部位和范围分为A、B、C、D四型，C型夹层因累及近心端和远心端可以分为Cp、Cd两个亚型。

3.主动脉夹层阜外分型：

A型：累及升主动脉，夹层终止与无名动脉近端。

B型：夹层局限于胸降主动脉，或延伸到腹主动脉，甚至进去髂动脉：但升主动脉及主动脉弓均未受累及。

C型：夹层累及主动脉弓，无论升主动脉和胸降主动脉是否受累及。其中包含两个特殊亚型：

Cp：夹层仅累及到主动脉弓近心侧的无名动脉和/或左颈总动脉，远心端未受累及。

Cd：夹层仅累及到主动脉弓远心侧的左锁骨下动脉和/或左颈总冬眠，近心端未受累及。

D型：夹层局限在膈肌以下，腹动脉受累的夹层。

阜外主动脉数据库（FWAdatabase）

例阜外主动脉夹层分布（～年），按照阜外分型进行区别可以看到比例最大的是C型（包括Cp和Cd，几乎占到2/3。B型夹层是例，单纯的A型夹层99例，D型夹层非常少，只有2例。

主动脉夹层根据阜外分型处理原则

A型：积极开放手术治疗，实施升主动脉置换，根部受累者，根据情况，一并处理。

（2）内脏器官、肾、肢体缺血；

（3）持续性腹痛治疗后缓解；

（4）主动脉直径＞3cm；

（5）肾平面上孤立性腹主动脉夹层动脉瘤。

Tsagakis改良分型（，德国）

Tsgakis分型又称为改良Debakey分型，主要对于DebakeyⅡ型的修改，DebakeyⅡ型由原来的仅局限于升主动脉，调整为累及到左锁骨下动脉近端，降主动脉不受累。早期由于技术限制，DebakeyⅡ型仅限于升主动脉，手术方式为升主动脉替换，由于主动脉弓置换术等技术的进步，改良DebakeyⅡ型包括升主动脉和部分主动脉弓。DebakeyⅡ型和DebakeyⅠ型在治疗方式和预后存在差异。

Penn分型（滨州分型）（，美国）

滨州分型主要针对急性StandfordA型夹层的细分，依据是否有局限脏器的缺血损伤和全身的循环衰竭可以分为Aa、Ab、Ac、Abc四型。

IRAD（国际主动脉夹层注册登记）分型（，美国）

IRAD根据不同时间死亡率将夹层分为四组：超急性期（0-24h），急性期（2-7d）、亚急性期（8-30d）、慢性期（≥30d）；更新慢性夹层（大于2周）的概念，提出亚急性期概念，发病30天内，患者的生存率随着时间的延长死亡率上升。对于低风险病人，延迟手术可以改善患者的死亡率。

1、此项研究是回顾性资料，存在一定数据缺失；并且处于研究初期，是治疗方式的一个演进过程。
